Two-step immunotherapy attacks advanced ovarian cancer

Jan. 31, 2013 ? Most ovarian cancer patients are diagnosed with late stage disease that is unresponsive to existing therapies. In a new study, researchers from the Perelman School of Medicine at the University of Pennsylvania School of Medicine show that a two-step personalized immunotherapy treatment -- a dendritic cell vaccine using patients' own tumor followed by adoptive T cell therapy -- triggers anti-tumor immune responses in these type of patients. Four of the six patients treated in the trial responded to the therapy, the investigators report this month in OncoImmunology.

"What we proved in this study is that this is a safe treatment strategy," says co-first author Lana Kandalaft, PharmD, MTR, PhD, research assistant professor of Obstetrics and Gynecology and director of clinical development in the Ovarian Cancer Research Center. "It is a walk in the park for patients, especially compared to standard chemotherapies and surgical treatments for ovarian cancer -- literally, some patients left the clinic and went for a walk in a nearby park after their treatment."

The findings follow research by the study's senior author, George Coukos, MD, PhD, director of the Ovarian Cancer Research Center at Penn, who showed in 2003 that women whose ovarian tumors were infiltrated by healthy immune cells, called T cells, tended to live longer than women whose tumors were devoid of T cells. That observation and other subsequent ones suggest the patient's immune system is trying to fight off the disease but can't quite muster the strength to beat it. Therefore, investigators have been trying to find ways using patients' own tumor cells to boost the immune system's power.

In the current study, Coukos, Kandalaft, co-first author Daniel J. Powell Jr., PhD, research assistant professor of Pathology and Laboratory Medicine, and colleagues treated six women with advanced ovarian cancer in a two-staged immunotherapy protocol in which they utilized a dendritic cell vaccine created from tissue in the patients' own tumor, which was stored at time of surgery. All of these women's cancers had progressed on standard of care chemotherapy.

In the first segment of the study, the team prepared an individualized dendritic cell vaccine for each patient. They harvested dendritic cells from each patient using apheresis, the same process volunteers go through when they donate platelets or other blood products such as those collected for stem cell transplants. Kandalaft and colleagues then exposed each patient's dendritic cells to tumor extract produced from the woman's own tumor, which teaches the dendritic cells who the enemy is. After this priming, the investigators vaccinated each patient with her own dendritic cells and gave them a combination chemotherapy regimen of bevacizumab and cyclophosphamide. Because dendritic cells are like the generals of the immune system, they then induce other immune cells to take up the fight.

Of the six patients who received the dendritic cell vaccine, four developed an anti-tumor immune response, indicating that the approach was working. One of those patients had no measurable disease at study entry because all of it had been successfully removed during surgery. She remains in remission today, 42 months following vaccine treatment. The other three who had an immune response to the vaccine still had residual disease and went on to the second segment of treatment.

The team harvested T cells from each of these three women. Using a technique developed at Penn, they grew the cells in the laboratory, expanding their numbers exponentially, and then reintroduced them into each patient after she underwent a lymphodepleting chemotherapy regimen. Because the T cells had already been trained by the dendritic cell vaccine to attack the tumor cells, the adoptive T cell transfer amplifies the anti-tumor immune response.

Two of the women showed a restored immune response after the T cell transfer. One of the women continued to have stable disease, whereas the other had a complete response to the therapy.

The researchers say it is too early to say whether this type of therapy will be effective in a large number of ovarian cancer patients, but the early results are promising. First, and foremost, she notes, the two-step approach appears safe and well tolerated by the patients. Additionally, the team saw a correlation in both treatment steps between immune responses and clinical benefit, suggesting that it is, in fact, the immune response that is holding the disease in check.

With these encouraging results in hand, the team has opened a larger trial in which they have already enrolled about 25 women and aim for up to 30 more. The new protocol uses an improved vaccine platform and an optimized adoptive T cell transfer protocol. The PI of this study is Janos Tanyi, MD, PhD.

"Large clinical trials have shown that intensifying chemotherapy doesn't improve outcomes for women with advanced ovarian cancer," Coukos says. "So we need to explore other avenues. We think the combinatorial approach of both immune and chemotherapy is the way to go."

Gene found that turns up effect of chemotherapy

Jan. 29, 2013 ? Chemotherapy is one of the most common treatments for cancer patients. However, many patients suffer from serious side-effects and a large proportion does not respond to the treatment. Researchers from the Biotech Research and Innovation Centre (BRIC) and Center for Healthy Aging, University of Copenhagen, now show that the gene FBH1 helps turn up the effect of chemotherapy.

The results were recently published in the journal Nature Communications.

"Our results show that the gene FBH1 is crucial in order for some chemotherapeutics to become active in the body and kill the cancer cells. If we can find a feasible method to increase the activity of the gene, we can use our cells' own resources to improve cancer treatment, says associate professor Claus S?rensen who has lead the team of researchers behind the results.

Own gene helps chemotherapy fight cancer

The researchers have used a method called RNA interference to study whether some of the genes in our DNA are important for cancer cells to react to certain chemotherapeutics.

"By using the method to remove single genes from cancer cells and then exposing the cells to chemotherapy, we found that FBH1 is important for the effect of the chemotherapy. Actually, the presence of the gene was an absolutely requirement in order to effectively kill the cancer cells with the type of chemotherapeutics we have studied, says postdoc Kasper Fugger who has led the experimental part of the investigation.

Chemotherapy act by exposing cancer cells to a kind of extreme stress when they divide. The result is detrimental damage to the cells' DNA that cannot be repaired, causing the cells to die. The new results show that it is in fact FBH1 that contributes to the formation of DNA damage when treating with chemotherapy and this knowledge can be used to optimize cancer therapy.

Selection of patients for chemotherapy

In the last decade it has become clear that targeted treatment to individual cancer patients is crucial for an effective treatment with least possible side-effects. By assessing the presence of FBH1 in a tumour the doctors can get an indication of whether the patient will benefit from chemotherapy.

"Our results could help indicate that patients with low or no FBH1 in the cancer cells will not benefit from certain types of chemotherapy, but should be administered another type of treatment. So by using the genetic fingerprint of a tumour doctors can adjust the treatment to individual patients, says Claus S?rensen.

The next step -- finding the FBH1 volume knob

The next step for the research team is to investigate the presence of changes, so-called mutations in FBH1. Identifying mutations rendering cancer cells resistant to certain chemotherapeutics can be used to target the treatment even better to individual patients. Another goal for the researchers is to find a way to turn up the activity of FBH1 in cancer cells.

"Our hope is to find a method to boost the activity of the FBH1 gene in cancer cells since this will make them more sensitive to chemotherapy. Alternatively, we may find a way to simulate an effect similar to that of FBH1, which can be used as additional treatment in order to sensitise cancer cells to chemotherapy. If we achieve this, more patients will benefit from the treatment, says Kasper Fugger.

Jan. 27, 2013 ? Charged particle accelerators have become crucially important to modern day life, be it in health care for cancer treatment or for answering important fundamental scientific questions like the existence of the HIGGS boson, the so called 'God particle'.

In a simple picture, charged particles like electrons and protons are accelerated between two end plates across which an electrical voltage is applied. High energies need high voltages (millions and billions of volts) and long acceleration paths in giant sized machines -- for instance the trillion volt machine called the 'large hadron collider' (LHC) which discovered the Higgs boson, circles over 27 km underground in Geneva! A new concept for a compact accelerator was discovered in the last decade using high powered, short pulses of laser light.

Alternating large electric fields of the light can be transformed in plasmas to create quasi static fields that can produce hundreds of millions volt accelerating voltages just over millimeter lengths on a table top!

How do we accelerate neutral particles -- i.e. particles that cannot be energized by electrical voltages? And do it over millimeters rather than hundreds of meters and moreover using lasers? Research at Ultra Short Pulse High Intensity Lab in TIFR has now found a novel scheme that can do precisely this. The concept uses the ability of powerful lasers to strip nearly 8 electrons per atom in a nano sized, cooled aggregate of argon atoms- a nano piece of ice. A 40,000 atom cluster of argon is charged to 320,000 by a laser that lasts only a 100 billionth of a millionth of a second. Such a super highly charged ice piece explodes soon after, accelerating the charged atoms (Ions) to a million electron volts of energy. The TIFR research now found that all the expelled electrons can be put back into the charged ion that has been accelerated so that it now reverts to being a neutral atom but at high energies. To top it all, this process is nearly 100% efficient at neutralizing the speeding ions and converting them to fast atoms.

Accelerated neutral atoms are very important for many applications. Unaffected by electric or magnetic fields, they penetrate deeper in solids than electrons/ions and thereby create high finesse microstructures for novel electronics and optical devices. Fast atoms are used both as diagnostics and heating sources in Tokomak machines like the ITER (International Thermonuclear Experimental Reactor) in France, that are being developed to create sustained thermo-nuclear fusion. The TIFR scheme can produce a point source of fast neutral atoms close to the location of an intended application.

This certainly shows that staying neutral under extreme provocation has its advantages.

Research by US Forest Service scientists forecasts profound changes over the next 50 years in the summer range of the endangered Indiana bat. In an article published in the journal Ecology and Evolution, Forest Service Southern Research Station researchers Susan Loeb and Eric Winters discuss the findings of one of the first studies designed to forecast the responses of a temperate zone bat species to climate change.

The researchers modeled the current maternity distribution of Indiana bats and then modeled future distributions based on four different climate change scenarios. "We found that due to projected changes in temperature, the most suitable summer range for Indiana bats would decline and become concentrated in the northeastern United States and the Appalachian Mountains," says SRS research ecologist Loeb. "The western part of the range (Missouri, Iowa, Illinois, Kentucky, Indiana, and Ohio)currently considered the heart of Indiana bat maternity rangewould become unsuitable under most climates that we modeled. This has important implications for managers in the Northeast and the Appalachian Mountains as these areas will most likely serve as climatic refuges for these animals when other parts of the range become too warm."

In general, bat species in temperate zones such as Indiana bats may be more sensitive than many other groups of mammals to climate change because their reproductive cycles, hibernation patterns, and migration are closely tied to temperature. Indiana bat populations were in decline for decades due to multiple factors, including the destruction of winter hibernation sites and loss of summer maternity habitat.

Due to conservation efforts, researchers saw an increase in Indiana bat populations in 2000 to 2005, but with the onset of white-nose syndrome populations are declining again, with the number of Indiana bats reported hibernating in the northeastern United States down by 72 percent in 2011. The study predicts even more declines due to temperature rises from climate change, with much of the western portion of the current range forecast to be unsuitable for maternity habitat by 2060.

"Our model suggests that once average summer (May through August) maximum temperatures reach 27.4C (81.3F), the climatic suitability of the area for Indiana bat maternity colonies declines," says Loeb. "Once they reach 29.9C (85.8F), the area is forecast to become completely unsuitable. Initially, Indiana bat maternity colonies may respond to warming temperatures by choosing roosts that have more shade than the roosts that they currently use. Eventually, it is likely that they will have to find more suitable climates."

The models the researchers produced provide resource managers guidance on areas that are likely to contain maternity colonies now and in the future, depending on the availability of suitable habitat in those areas. "Managers in the western parts of the range should be aware of the potential changes in summer distributions due to climate change and not assume that declines are due to habitat loss or degradation," says Loeb. "Management actions that foster high reproductive success and survival will be critical for the conservation and recovery of the species."

Velayati, Iran's former foreign minister, said the Lebanese militant group Hezbollah and the Palestinian Hamas would had been defeated in wars with Israel without Syrian support.

"If Syria didn't provide logistical support to Hezbollah, Hezbollah and Hamas would have not achieved victory in the 33-day and 22-day wars (respectively)," he said.

He was referring to the 2006 Israel-Hezbollah War and Gaza War during the winter of 2008-2009. Both militant groups claimed victory although Israel has disputed this.

"Regional reactionary (regimes) and the West attacked the golden ring of resistance by targeting Syria," Mehr quoted him as saying, a reference to U.S. and Gulf Arab backing for the rebels.

Iran is Syria's most important ally in the Middle East. Tehran has provided Assad's government with military and political backing for years, and has kept up its strong support for the regime since the uprising began in March 2011.

They then explored the reason why the Austrian physicist arrived at an energy/mass correlation with the wrong factor, namely at the equation: E = (3/8) mc². Hasen?hrl's error, they believe, stems from failing to account for the mass lost by the blackbody while radiating.

Before Hasen?hrl focused on cavity radiation, other physicists, including French mathematician Henri Poincar? and German physicist Max Abraham, showed the existence of an inertial mass associated with electromagnetic energy. In 1905, Einstein gave the correct relationship between inertial mass and electromagnetic energy, E=mc². Nevertheless, it was not until 1911 that German physicist Max von Laue generalised it to include all forms of energy.

WASHINGTON (AP) ? President Barack Obama will nominate Mary Jo White to lead the Securities and Exchange Commission, tapping an attorney with broad experience in prosecuting white-collar crimes to lead an agency that has a central role in implementing Wall Street reform.

A White House official said the president would announce White's nomination during a ceremony in the State Dining Room Thursday afternoon.

At the same event, Obama will renominate Richard Cordray to serve as head of the Consumer Financial Protection Bureau, the official said. The president used a recess appointment last year to circumvent Congress and install Cordray as head of the bureau. That appointment expires at the end of this year.

The official spoke on the condition of anonymity in order to discuss the nominations ahead of the president.

White spent nearly a decade as the U.S. attorney in Manhattan, building a reputation as a tough prosecutor with an expertise in pursuing white collar crimes and complex securities and financial fraud cases. White House officials say that experience makes her well positioned to implement Obama's Wall Street reform legislation.

While serving as U.S. attorney, White also won convictions related to the 1998 bombings of two U.S. embassies in Africa and the 1993 World Trade Center bombing.

If confirmed by the Senate, White would take over the helm at the SEC from Elisse Walter, who is serving out the rest of former SEC chair Mary Schapiro's term. Schapiro resigned in December.

Cordray has run the consumer bureau since last year, when Obama used a recess appointment to install him in the job. Senate Republicans had opposed Cordray, as well as the concept of the consumer bureau.

Sen. Elizabeth Warren, D-Mass., first conceived of the idea of a consumer protection bureau. Obama considered naming her to lead the bureau, but her nomination would likely have run into deep opposition on Capitol Hill.

White, 65, currently heads the litigation department at law firm Debevoise & Plimpton.

She was the first woman to hold the position of U.S. attorney in Manhattan, one of the most prestigious positions in federal law enforcement. During her tenure from 1993 to 2002, White won convictions of white-collar criminals, drug traffickers and international terrorists. The most notable was Ramzi Yousef, the mastermind of the 1993 World Trade Center bombing.

She also led the prosecution of mob boss John Gotti when she was acting U.S. attorney in Brooklyn in 1992. Gotti died in prison in 2002.

If confirmed by the Senate, White would be the first prosecutor to head the 79-year-old SEC. Most SEC chairmen traditionally have come from Wall Street or the ranks of private securities lawyers. The choice of White is likely intended to bolster the agency's enforcement profile in the aftermath of the financial crisis.

White's background differs sharply from Schapiro, who stepped down last month after guiding the agency in the four years after the crisis. Schapiro worked at the Commodity Futures Trading Commission and the Financial Industry Regulatory Authority, the securities industry's self-policing organization. Some consumer advocates have said that Schapiro's experience as CEO of FINRA made her more likely to seek compromises and less likely to aggressively pursue misconduct.

During Shapiro's tenure, the SEC reached major settlements with the biggest banks on Wall Street, including Goldman Sachs, JPMorgan Chase and Citibank. But critics said the penalties were small compared with the banks' revenues. And they complained that no senior executives were held accountable.

White would be expected to give high priority to expanding the enforcement efforts.

At the same time, much of the pressing work facing the agency involves writing new rules. The SEC is seeking stricter rules for money-market mutual funds and must get into shape the so-called Volcker Rule, which would bar banks from making certain trades for their own profit.

As head of the litigators at Debevoise & Plimpton, White has represented a number of financial institutions likely to have crossed swords with the SEC in enforcement cases. Her clients also included former Bank of America CEO Ken Lewis, whom she represented in a 2010 civil lawsuit by then-New York Attorney General Andrew Cuomo accusing Lewis of misleading shareholders in the bank's merger with Merrill Lynch.

White also represented the largest U.S. hospital chain, HCA, in the insider-trading investigations by the SEC and the Justice Department of former Senate Majority Leader Bill Frist, whose family owned HCA. The investigations were closed in 2007 with no charges filed against Frist.

This small group would remain available as a contingency should some unforeseen disease emerge for which chimps would be the best stand-ins for humans. But they, along with the retirees, would be housed in facilities designed to more adequately accommodate the full range of normal chimp physical and social activities ? from climbing, foraging, and daily nest-building to hanging out in sizable groups on branches high off the ground, according to the panel.

The panel also recommends ending 16 of 30 research projects involving chimpanzees that the NIH currently is funding. The largest proportional hit falls on biomedical research, one of three categories of projects. Six out of nine current biomedical projects would end.

The ultimate driver behind the recommendations: concerns about the value and ethics of using chimpanzees, biologically the nearest relative to humans, for physically painful and intrusive infectious-disease research.

If the 28 recommendations are implemented, the effort would represent "an historic step forward" in moving chimps out of the lab and into sanctuaries, says Kathleen Conlee, vice-president for animal-research issues at the Humane Society of the United States, based in Washington.

Even foes of federal legislation to greatly restrict the use of chimps and other "great apes" in biomedical research see merit in the new recommendations.

As a stand-in for humans, "the chimpanzee has played a very important role in the evolution of biomedical research," notes Frankie Trull, president of the National Association for Biomedical Research (NABR) in Washington, which fought against the Great Apes Protection and Cost Reduction Act of 2011, which died in December with the end of the 113th Congress.

But biomedical science has advanced, Ms. Trull continues. And keeping chimpanzees is expensive; chimps are not euthanized but must be cared for until they die naturally. Researchers have found alternative animal models for some of the kinds of studies that once centered on chimps.

Although NABR opposed the Great Ape Protection and Cost Reduction Act, the group is comfortable with the recommendations the NIH is now considering, Trull says.

The case for change and the steps to take came from the scientific community, she observes, adding, "scientists should determine what animal models should be used, not Congress."

Chimpanzees represent a tiny proportion of animals used in biomedical research. The overwhelming majority of animals used are either rodents or zebra fish.

The recommendations represent the outcome of a process that began at the end of 2010, when three US senators asked the US National Academies to examine the issue, as did the NIH. A year later, the National Academies' Institute of Medicine released its report.

The 86-page report released for comment on Jan. 22 was pulled together by a senior working group that the NIH gathered to turn the Institute of Medicine's report into specific recommendations.

If adopted, the recommendations would apply only to NIH-owned chimps and those used in the course of NIH-funded research. Of 670 chimps the NIH owns or supports, 219 have been retired. Some 282 are research-ready. Another 169 have been labeled "research inactive," a kind of bridge category between the first two.

By some estimates, another 350 chimps would fall outside the purview of these recommendations because they are owned either by private pharmaceutical companies or by universities.

Indeed, the Human Society's Ms. Conlee suggests the Great Ape Protection Act is likely to be reintroduced this year to broaden restrictions to chimps not covered by the new recommendations.
